Gastric Bypass Myth - All Patients Stretch their Stomachs and Regain Weight

By Kaye Bailey Have a conversation about gastric bypass surgery with just about anyone and you will hear, Yeah, I know someone who had that done and within a year they stretched out their stomach and regained all that weight plus some. Unfortunately it is true that some weight loss surgery (WLS) patients do regain their weight after losing it. What isnt true is that they stretch their stomachs back to pre-surgical size. At best, a post gastric bypass stomach will expand from a capacity of 2 tablespoons to one-cup capacity. This is expected and part of the reason gastric bypass is successful. In the phase of rapid weight loss the patient cannot eat more than once ounce of food at a time. As the stomach heals and the weight loss stabilizes the stomach can eventually hold up to a cup of food at a time. The reason that some patients regain their weight after surgery is they return to snacking which is contradictory to the directions given by their bariatric center. Snacking is forbidden by most centers. Eating little quantities of the wrong foods throughout the day causes WLS patients to stop losing weight, or worse, this behavior results in weight gain. Snacking is one of the behaviors that caused morbid obesity in the first place. Snacking is the downfall of the WLS patient who regains weight, not stretching the stomach. For example, at five years out of surgery, I can eat one piece of pizza at dinner. If I eat a second piece at dinner I WILL get sick. Guaranteed. However, If I nibble on the leftovers an hour later, I can add another piece, and an hour later, another piece, and so-on. See the pattern? Snacking is the problem, not stretching the stomach. The fact is, patients who live by the four rules do not regain their weight. The four rules for long-term success after gastric bypass surgery are: Eat protein first; No snacking, Drink lots of water and Exercise daily. The four rules are in place to ensure successful weight loss and long-term weight maintenance. By Marsha Maung The rebel years are over. The phase of contradiction and compulsive behavior comes to a grinding but definitive halt. The act of pure impulse sometimes becomes impossible. Youre older. I must admit, even in front of my own parents, I have admitted that I have been nothing short of a hell-raiser. Running away from home, staying over at boyfriends face, kicking the hell out of my own sister, engaging in drunken cat-fights with friends, fist-fighting with my brother, word-battles with my father, thinking I was right when I was irrefutably wrong..the list can go on. Whatever a daughter can do wrong, I did. But these are the activities, decisions, and memories that have helped shape me into the person that I am today. I have 2 wonderfully amazing and perfect boys to call my own. They look up to me, adore me and even when I think I am a little psycho, they think I am hilarious! The innocence..I am not yet a perfect person today but I can proudly say that I have become closer to perfect….in my personal opinion, that is. Age does this to people. When people think I should keep a job, I left it. When people think I should not be in a relationship, I engaged in. When people thought I should just shut up, I spoke up. When people thought I should be more feminine, I kick out and punch around like a crazy woman on drugs! When people thought I should forgive, I am revengeful. When people think I should forget, I remember. Gosh, when people think I should remember (like where I placed my keys), I dont. With that said, I look at my own parents and wonder how many of my decisions have made them into the older people that they are. How many of my rebellions have added an extra crease to their foreheads? How many of my shouting matches have given my parents wrinkles and white hairs? Only when youre older, you realize that Heck, I wasnt such a smartass, was I? and there I was, all of 16, thinking I was adult-enough to make my own decisions. If I had a daughter like me, I would have done things to her (and/or myself) that I will live to regret!Thankfully, I dont. I dont have a daughter, period!! (Someone up there loves me, after all) I spoke very briefly on the phone with my aging and lonely grandmother today and this blog is a result of that conversation. To say I have regretted my actions and decisions when I was younger is an understatement. We all dont know how long she has to live on this planet anymore but one thing is for sure, it wont be for long. Oh, shes not really ill or anything. Shes happy (in a very lonely kind of way) and healthy (in an old kind of way) but she certainly has her own regrets as well. I guess a lot of the things that I did in the past were uncalled for and when she did things out of the goodness of her heart, I wasnt appreciative because I was too self-centered and obnoxious. I ruled the world, didnt I? I dont know how long more the tenure of her stay here on earth has before it expires, but I hope she will take good memories with her down or up to wherever shes going after the expiration of her stay here. Before I go any further, this article is not about selling your body for a cool $1million although Im sure an article like that would spark a lot of controversy. There is a quote in the movie which really struck a chord with me, the essence of it is: if you love something, set it free if it comes back to you, it is yours forever. If it doesnt come back to you, it was never yours to begin with. This quote has had a very profound effect on the way I look at relationships. I used to be petrified of losing a good thing when I had it, so scared that I used to hold on to it very tightly, just in case is slipped through my fingers. The problem is: When you hold on to something so tightly, it cannot breathe a relationship needs to breathe in order to grow. It took me a long time to come to the realisation that I didnt need to hold on so tightly. The problem you have when you spend all your of energy trying to keep hold of something, is that you have no energy left in order to enjoy it. What is the point in having something that you cant enjoy? Here is another way to think about it How much more beautiful is an animal in the wild? Think about that for a moment. Most people prefer to see animals in their own natural environment, obviously, the animal enjoys it too. It is the same in a relationship, it is all too easy to feel trapped. When we feel trapped it is a natural instinct to try to escape, that makes sense doesnt it? Now, on the other hand when we feel free, we are able to make our own choices and we tend to gravitate towards the things in our lives which make us feel good Now, who would you rather be with; somebody who wants to lock you up in a cage, or somebody who gives you the space and emotional freedom you need?What Im really trying to say is this: Are you frightened of losing something in your life? By Antonella Novi As a child, from three to nine years old, I had the pleasure of attending Kiers Kids N Stuff. In Martinez, California- the San Francisco Bay Area- it was the best day care around. Mrs. Kier had lots of qualified help, thus my sister Laura and I had many friends. To this day, 20 years later, I am still best friends with Kristen Dippo, my first friend ever. My favorite memories of my day care experiences are of doing art projects. Whether it was working with the salty clay, or simply coloring, I enjoyed the creative process. I loved bringing home my completed art so Mom and Dad could put it on display. I grew up and we moved to Washington. I continued to enjoy art, especially the natural flow of watercolors. Since I was young I took my time with my art pieces, so its no wonder I didnt meet the set deadlines in art class in high school, thus receiving a poor mark. While in college I used art as a means of de-stressing after a hard week of studying. One class I took incorporated natural environmental sculpture into the curriculum. It was fascinating to use the earth as my easel and its stones, leaves, and twigs as my tools for expression. Upon returning to Anacortes, I met Monica Oppel, the Arts and Crafts Coordinator, or the Art Lady of the Boys and Girls Club here in Anacortes. She told me it would be nice to have some help. So, in the summer of 2002 I joined her and about 15 kids for Art in the Park. Together we helped kids from ages 6-15 produce natural art through photographic paper (sun prints). As I helped the kids, I remembered myself when I was their age- curious, imaginative, and creative. Each piece of art produced was so different from the next. Each beautiful in its own way. I was excited to Volunteer the next day to see what the kids would make next with Monicas idea and my help. I will never forget the project the next day: Plant squishing (flower pounding) onto special absorbent fabric. This time I did the project with the kids. I asked THEM to help ME. A six year old boy handed me what flowers he thought would press well. The outcome was truly beautiful! At first I could only help The Art Lady at the Boys and Girls Club for an hour at the max. My tolerance and patience level was not strong enough to handle 20 kids all at once. But as the weeks and months wore on I built up stamina and courage. Now the longer I stay in the presence of the children the more fun I have. I learn from them, and I have become patient and aware of their creative needs. I now work two or three days a week with Monica. I even go in early to help her set up for projects, and sometimes stay long enough to help her clean up the messes. The Art Room is in a quiet nook, kind of separate from the rest of the Boys and Girls Club. The room is peaceful; we listen to relaxing music or show tunes. Some of the kids are regulars, while others rotate in and out. On a normal day The Art Lady and I usually handle from 10 to 30 kids. The art projects are different everyday. Sometimes we have three different projects going at once depending on their level of difficulty. Origami has been very popular recently. I have taken to sitting at the childrens level at the tables, and helping them one on one. I talk to them about their day at school and their families. I share my stories of what I was like when I was their age and what its like to be a young adult. We enjoy talking about animals, since I pet-sit a lot. I have many funny stories about the animals in my care. I have noticed that if I work on the project with the kids instead of just telling them what to do, I have more energy, and I have more fun too. So scattered around my home are remnants of my weekly rendezvous in the Art Room. I have a bean mosaic, three collages, a colorful woven rag rug, a hand-made picture frame with positive words, a salted watercolor, painted ceramic magnets, and I am sure there are more goodies that I have passed on to friends. I framed two art pieces made by Chloe (a bunch of leaves in the pattern of a frog) and Amanda (a princess). Everyday that I attend the Boys and Girls Club Arts and Crafts Room is like an adventure. It is a time warp back to my childhood. A chance to be creative, yet helpful. First I would like to say a personal thank you for taking the time to read this article. A question you will inevitably ask is, Does SSA look at my forms and can they alone win or lose my case? My experience is that SSA and/or judges dont usually approve your case based on what you say on the forms. However, they often use what is said in the forms to support a denial of your claim. This is because if SSA or a judge is going to approve your claim, they will base it on more compelling objective evidence such as medical records and/or treating physicians opinions regarding your inability to work. Unfortunately, I have seen what appeared to be an innocent statement by a claimant be the evidence used by the judge to deny the claim. The inherent problem you have as a claimant is twofold. Tip #2 Presume you are having a bad day when providing answers Remember, a critical issue in a social security disability case is always what activity level are you capable of sustaining on a regular and continuing basis (i.e. a 5 day work week). The issue is never what you can do for only one day. Clearly, almost everyone is capable of performing some activities for one day such that it would make them appear to be capable of working. Never forgetthe issue is always what level of activity you can sustain on a daily basis, week after week. Why should you assume you are having a bad day? Simply put, if you were back working on a sustained basis, most likely every day would be a bad day. Answering as if youre having a bad day is not only an honest answer but is also a more accurate assessment of what activity level you can sustain. Following this tip will avoid the problem of overestimating what you are capable of doing. It will also keep your denial mechanism honest. Tip #3 The Big Three - Always focus on the frequency, severity and duration of your symptoms and limitations Another critical issue in a social security disability case is your symptoms and limitations (i.e. pain, fatigue, concentration problems, inability to maintain any activity for a reasonable period). Always remember, you are unable to work due to the frequency, severity and duration of your symptoms and limitations, and not due to a diagnosis. You should mention all the diagnoses that have even a small impact on your inability to work, but you should use 5% of the allotted space to reference diagnoses and 95% to discuss The Big Three and how they limit not only your ability to work but also your ability to function on a daily basis. Tip #4 Completely resist the urge to be the perfectionist you are! Before you became ill you were probably an organized perfectionist who was incredibly productive. Everything in your life had its place; I know it kills you it is not that way now. However, this is not the time to be a compulsive, organized perfectionist! Please dont use SSA forms as a launching pad to organize your life. One of the hallmarks of your inability to work is your concentration problems, memory impairment and brain fog. Your life is now an unorganized mess. Guess what? SSA needs to see the real you and not a top notch administrative assistant who is articulate and possesses phenomenal organizational and typing skills. Please do not typewrite your answers. Always handwrite them even if your answers become illegible. The clarity of your handwriting and the way you answer the questions tells a lot about the severity of your concentration and memory problems. Many clients tell me that little by little, it took them days to complete some SSA forms. Your goal should be to have your answers look like it took you days. In fact, if it did take you days, make sure you tell SSA that somewhere on the form. I have an interesting story for you on this subject. Several years ago I represented a woman who was at one time an outstanding legal secretary. We eventually won her claim before a judge. However, before she retained me and prior to the judge, her claim was actually recommended for approval at an earlier level of review by SSA. Unknown to the client, SSA overturned its own decision and denied her claim. Why? Because she did a marvelous job typing her answers on the forms. There were no typos and the forms were perfectly organized. A reviewing SSA psychiatrist was so impressed with her work he wanted to hire her! He concluded her memory and concentration problems could not be as severe as she was alleging. His opinion was the forms proved she was capable of performing a simple, sit down job and her claim was denied. We were fortunate to have a judge who overlooked the forms and instead listened to her entire story. However, you may not be so fortunateso dont take any chances. Tip #5 If psychological issues play even a small part in preventing you from working, you must allege them on the forms Although the primary reason you are unable to work may be due to a physical diagnosis, dont overlook the psychological issues that often arise after years of dealing with chronic pain and fatigue. You want to win your case anyway you can, whether it is due to physical or psychological problems, or quite frequently, a combination of both. My experience in successfully representing chronic pain and chronic fatigue clients all over the country suggests that psychological issues are frequently a significant factor in why many people are unable to work. To totally ignore this fact can be detrimental not only to your well being, but may also cause you to lose your SSA disability claim! A Social Security judge recently told me that he hates it when a disability attorney fails to give me several doors to use if I want to approve a case. He believes that using only one door for claim approval is tantamount to the attorney driving their client off a cliff. I totally agree with the judge. Please dont let this happen to you; you have too much at stake! What did the judge mean? Simply put, judges like to have several medical conditions to choose from if they want to approve your claim. For example, if a judge believes your claim is not strong enough to be approved based only on your physical diagnosis, they want the option to approve your claim based on another diagnosis, perhaps psychological. What prevents the judge from simply approving it based on a psychological diagnosis? The problem is you never told SSA on any of the forms you completed that you believed a psychological condition was at least in part responsible for why you cant work! Thus, a door that could have used to approve your claim does not exist; consequently, the judge has no choice but to deny your claim. This scenario is terribly unfortunate because your claim was denied when the judge was looking for a way to approve it! The solution is to tell SSA early, often and consistently that you believe a psychological diagnosis plays a part in your inability to work. It is fine to state that it is secondary to or as a result of dealing with your chronic physical symptoms and limitations. At the end of the day, you want to win your case anyway you can. I know this is true because all over the country I see it in my clients eyes on the day of the hearing. SSAs monthly benefit check and health insurance benefits mean the same to you regardless of what diagnosis was used to approve your claim. Remember, proper preparation as well as understanding what you need to prove and how you need to prove it are critical to winning your case.
